This application is for restaurants and qualified community organizations who want to participate as a food concessionaire at the Re-Discover Kailua festival. Please print out these instructions, then submit the requested information via e-mail, fax, mail or deliver in person to the address listed below.
QUALIFICATIONS - Food vendors must be able to: · Produce and serve a large volume of high quality menu items over a two day period from 10am to 10pm.
· Provide a certificate of general liability insurance naming the City & County of Honolulu and the Kailua Mustangs Pop Warner Association as additionally insured.
· Provide a copy of its business General Excise Tax License
· Provide a Temporary Food Establishment Permit certified by the Dept. of Health
RULES – Food vendors are required to:
· Keep their concession, tools and supplies in a clean and orderly state throughout the two day event
· Provide own printed menus (no hand-drawn menus) including prices for each item
· Provide enough labor to efficiently prepare, serve and collect cash throughout the two day event
· Provide a banner indicating the name of its restaurant/organization, extra electrical extension cords as needed, and other tools and supplies not provided by the event organizers
· Sign a Vendor contract agreeing to comply with the terms and conditions set forth by the Re-Discover Kailua organizers.
SELECTION CRITERIA – First consideration given to vendors who:
· Can produce high quality food at reasonable prices for the general public (suggested retail $3.00 - $7.00)
All applications are subject to selection by the Re-Discover Kailua Steering Committee. Vendors may be asked to change or adjust proposed menu items to avoid duplication.
